MODEL RELEASE AND GRANT OF RIGHT TO 

USE NAME AND PHOTOGRAPHS AND/OR VIDEO

The undersigned, _____________________________________, (referred to as GRANTOR),  hereby grants to <photographer's name> (hereafter referred to GRANTEE),its legal representatives, successors, and all persons or corporations acting with Grantee’s permission, the  unrestricted right to copyright and/or the  use of Grantor’s professional and/or stage name, and the right to publish photographic portraits or pictures of Grantor and  the unrestricted use of negative, transparencies, prints, or multiple, moving or video format, or in which Grantor may be included in whole or in part or composite or distorted in form and any and all rights to reproduction thereof, in color or otherwise, made through any media and for any other lawful purpose.



Grantor hereby waives the right to inspect or approve the finished product or copy that may used in connection  with any photographic image  previously delivered to Grantee by Grantor in connection with Grantee’s business.

Grantor further releases Grantee and all others, including agents acting under authority of Grantee, from any and all claims for remuneration as a result of Grantee’s use of Grantor’s portraits or pictures an any format as identified hereinabove and further released Grantee from any damages associated with such use, foreseen or unforeseen, so long as used in accordance with this Release Agreement.



Grantor hereby certifies by executions of this Release Agreement that Grantor is as least Twenty-one (21) years of age and that Grantor is authorized to execute this Release Agreement. The release granted hereunder is of perpetual nature and shall have no time limit for termination.
