Model Release Form

As the Photographer < Insert Name Here > I have requested the services of
Model < Insert Name Here > based on one of the pay scales outlined below.
All payments will be made in full the day of the shoot unless otherwise negotiated.

As the Photographer my rights to the pictures include using them for my own personal, non-commercial use. Any other projects that the photos will be used in must be negotiated with the Model before use. I agree to contact the Model before such use and if I fail to do so I can be held liable. The Photographer will hold all negatives unless otherwise outlined.
