formats whether now known or hereafter created.  Additionally I 
consent to the use of my name and likeness for any and all shows,

produces, or is somehow connected with.

Model releases (agency or photographer’s name), its’ affiliates, 
partners, directors, officers, employees and all others associated 
with the development and execution of this contest from any and all
liability, from injury, loss or damage of any kind arising out of 
or resulting from model’s presence throughout the contest, partici-
pation, appearance in, and use of any contest photographs thereof.
